An Athlete Medical Profile form should be printed, completed and submitted to the team trainer prior to the season or a player's first game.

The profile ensures that any relevant medical conditions of players are known to the trainer and that the trainer has access to parents’ contact information.

The Athlete Medical Profile is kept confidential.
