Team Managers should download, print and cut the form into three separate papers for each round.

Three different people (including the coach) should provide votes with 5 votes going to the player judged by that person as the best player on the day, 4 votes to the second best player and so on.

The votes should be sealed in an envelop marked as 'VOTES' with the following details also written on the envelop:

• Team Name
• Name of Round
• Date of Round.
